Exclusions and Limitations

• Cleaning: Limited to every 6 months
• Full Mouth X-rays & CBCT: every 25 months
• Perscriptions and OCT NOT included
• CANNOT have other dental coverage
• Health restrictions may apply
• Payment due at time of service
• Major services are limited to 3 month payment plan

Cancellation Policy

• Non-compliance of treatment
• 2 broken appointments (unless valid proof of reason)
• 1 missed payment if on a payment plan
